Conditioning

November 1st and 2nd :  3:00 – 5:00 @ Liberty Junior
November 6th, 8th, 9th and 10th :  3:00 – 5:00 @ Liberty Junior
November 10th will be the first practice


Athletes must have physical turned into the athletic office prior to participation.

All participation forms can be filled out online at lakota.finalforms.com after July 1.  All forms must be completed prior to first day of practice.  Click on link (red link above is a live link) and follow instructions.

In order to Participate -

Before any athlete can participate in Lakota athletics, they must have:

A current physical, signed & dated, by a physician on file with our office. (Physicals are valid for one calendar year.) Parents may obtain a physical from their doctor or Urgent Care facility. (Call the athletic office for Urgent Care locations.)

An Emergency Medical Authorization Form signed and dated, by parents/guardians

Updates: For more information on specific times, updates, and/or obtaining necessary forms needed before tryouts, please visit Liberty Junior’s homepage and click on “Athletics." Please be sure to double check dates and times as items may change during the spring or summer.

Physicals
State guidelines mandate that all athletes must have a physical before trying out for any team. It is important to note that physicals are only valid for one year from the issue date; therefore to ensure your child meets the requirements for the entire school year, we recommend that you schedule this physical in May, 2017. The physical form is located on the Liberty Athletic website (look now at the Quick Links--left sidebar--OHSAA Physical Form and click to get the form downloaded at your home) as well as in the front office. 

Athletic Fees
Athletic fees for the 2017-18 school year will be $150.00 for junior school students. A family cap is available for junior high and high school students participating in extracurricular athletics and marching band. See the Frequently Asked Questions and Family Cap Form for details.

IF YOU FEEL THAT YOU WILL BE ELIGIBLE FOR THE COMMUNITY FOUNDATION SCHOLARSHIP. Please fill out the proper form and submit it before Nov 1st otherwise you will not have that option. (This is not a guaranteed thing so please try if you absolutely need it).
